Hello, future Ecology diver. The following required documents provide important information regarding your upcoming Aquarium Ecology Specialty Courses at the Denver Downtown Aquarium.
Once you’ve read and understand the information on each document, you’ll need to print, initial, sign, and date those areas as indicated on each document. Your A-1 Instructor will collect and review the completed documents when you arrive.
UNFORTUNATELY, YOU WILL NOT BE ABLE TO PARTICIPATE IN THE AQUARIUM DIVES UNTIL THESE FORMS HAVE BEEN COLLECTED AND VERIFIED FOR COMPLETENESS AND ACCURACY

VERY IMPORTANT REGARDING THE DIVER MEDICAL/PARTICIPANT QUESTIONNAIRE
“If you answered YES to questions 3, 5 or 10 OR to any of the questions on page 2, please read and agree to the statement by signing and dating it AND take all three pages of the Participant Questionnaire and the Physician’s
Evaluation Form to your physician for a medical evaluation. Your participation in a diving course requires your physician’s approval. It must be signed by an MD or DO.
Unfortunately, you will not be allowed to participate in any in-water activities until the doctor signed medical form has been submitted to the instructor teaching your class. Once again, this is ONLY REQUIRED should you answer YES to questions 3, 5, or 10 above OR to any of the questions on page 2 of the Diver Medical/Participant Questionnaire.
