Important Stress and Rescue Documents

Hello fellow diver.  The following documents provide important information regarding your upcoming Stress and Rescue Course.

Once you’ve read and understand the information please print, initial, sign and date those areas as indicated on each document. Your A-1 Instructor will collect and review the completed documents when you arrive.

REQUIRED DOCUMENTS

  • Show proof of being an SSI Referral diver, Open Water diver or hold an equivalent certification from another recognized training agency
    Unfortunately you will not be able to participate in the program without showing the A-1 professional proof of certification and presenting the following (completed and signed) documents.
  • Be at least 12 years old
  • Have a MySSI account.  Be sure to upload a digital photo to your account (you’ll need this in order to access your Stress & Rescue training material)

The A-1 instructor will collect the following (completed) documents at the beginning of the first session.  Unfortunately, participants will not be able to continue until the instructor has reviewed them for accuracy.

(Note: The Physician’s Approval To Dive ONLY needs to be signed by a physician if any question on the Medical Questionnaire is answered with a ‘YES’.)

Unfortunately, you will not be allowed to participate in any in-water activities until this signed form has been submitted to the instructor teaching your class. Once again, a physician’s signature is ONLY REQUIRED should you answer YES to any question on the SSI Medical Questionnaire above.
