Why/when do I need a doctor’s sign-off on my Diver’s Medical Participant Questionnaire?

Safety is our number one priority. Before you jump into any in-water training, you will need to complete a Diver’s Medical Participant Questionnaire.

A physician’s sign-off is required in the following scenarios:

• You answer “Yes” to any medical questions: If you check “Yes” to any health conditions on your medical questionnaire, you must receive a physician’s approval before any in-water work can begin.
• Your medical condition changes: If you previously filled out a form but your health history has since changed—such as a new medication, recent surgery, a new diagnosis, or any other changes to your health—your prior approval is no longer valid. You will need to fill out a new questionnaire and obtain a new doctor’s sign-off that reflects your current medical reality.

⚠️ Important Note on Sign-offs:

Medical approvals must be signed by a licensed prescribing physician (MD, DO, NP, or PA). This signed document must be verified and uploaded to your MySSI profile before you can start any in-water training.
